On September 15-16, the Chautauqua Tapestry System of Care will host a two-day learning conference on evaluation research and social marketing. FidelityEHR's Matthew Schubert will present how electronic health record software can help reduce drift from high fidelity Wraparound and System of Care practice models.
"There are tremendous opportunities to improve care coordination and outcomes while saving time with the right tools," said Matthew Schubert. "Evidence-based care coordination software should track meaningful data points in real-time, but it's also important that this tool gives each team member a voice and place to share ideas and tasks. These pieces help to ensure that the Plan of Care is working effectively while streamlining and strengthening teamwork."
The conference will also cover:
- Social marketing planning
- Strategies and tactics in social marketing
- Persuasive storytelling
- Strategies for System of Care evaluation
- Best Practice Strategies for Engaging Youth and Shifting the “System"
- Utilizing whole school and human service agency data to evaluate what works and for whom
- Trauma-Informed Systems of Care
The conference will take place at the Chautauqua Institution in New York and registration closes on September 1. Organized by Chautauqua Tapestry System of Care, the conference is supported by the School of Social Work at the University of Buffalo and the Social Work Program at the State University of New York at Fredonia. The conference is also presented through a partnership with the TA Network, Westat and the Caring for Every Child's Mental Health Campaign.
